The Adams County Severe Weather Activation Plan (SWAP) is a severe weather response pilot program in collaboration with Adams County Government, ACCESS Housing, and Almost Home. 

SWAP will assess and administer hotel/motel stays when the weather poses a threat to the well-being and/or lives of people sleeping or living outdoors during the winter months.


SWAP will activate when conditions reach 32°F and below and wet, as well as 20°F and below and dry. Once conditions have been reached or are anticipated to be reached, SWAP will activate.


SWAP will serve people experiencing literal, unsheltered homelessness in Adams County, Colorado, prioritizing single adults, couples without children, and unaccompanied youth, families and people with pets. 


In the 2019 Point-In-Time Count, almost of one-third of people who were counted identified as unsheltered in Adams County (139 or 29% of the total count, total count = 483). Of the unsheltered, 94% were single adults without children. 72% shared that they have either a disabling condition, mental health, substance use, and/or a chronic health condition.

There are three main methods community partners and folks experiencing homelessness will hear about the SWAP activation status:


Community partners will receive an email notification by 10:00 AM when SWAP is activated. To receive emails, enter your contact information below. 


Anyone and everyone can call into the SWAP phone line to learn about the status of SWAP. The SWAP phone line number is (720) 523-6646 and will be updated by 10:00 AM daily.


The  ACCH website  will post activation updates. When activated, the SWAP status will be updated by 10AM.
